Jacob Kelly Fitness is a fitness-coaching business and is not a healthcare provider. Ordinary coaching records are not represented as being maintained under the Health Insurance Portability and Accountability Act, commonly known as HIPAA.

Please do not submit medical information that is unnecessary for evaluating or delivering fitness coaching. Seek assistance from a licensed healthcare professional for medical diagnosis, treatment, physical therapy, medical nutrition therapy, medication management, or other healthcare needs.

The FTC advises businesses collecting consumer health information to consider obligations under the FTC Act and, where applicable, the Health Breach Notification Rule in addition to HIPAA-related questions.
